- Those who received 1st dose at the Kirkwood Community Center on May 7th do not need to register.

Ages 12-15 Parent or Guardian must be present when vaccinated 

Ages 16-17 We will vaccinate if the consent form is signed in all required spots by Parent or Guardian. Consent form
